Treat early Lyme with oral doxycycline

Doxycycline is the first-line antibiotic for early Lyme disease in adults and children over 8 years. Take 100 mg twice daily for 10-14 days (adults) or 4 mg/kg daily in two divided doses for children (max 100 mg per dose). Most patients see resolution of erythema migrans within days.

Why It Works

Doxycycline achieves high tissue penetration and is bactericidal against Borrelia burgdorferi. It also covers common co-infections like Anaplasma phagocytophilum, making it the preferred choice in endemic areas. The 2020 IDSA/AAN/ACR guidelines strongly recommend it as first-line therapy.

Tips

  • Take with food and a full glass of water to reduce nausea
  • Avoid prolonged sun exposure during treatment (photosensitivity risk)
  • Short courses under 21 days are now considered safe for children of all ages
  • Do not take with dairy products, antacids, or iron supplements (reduces absorption)
